Over time, abilities and needs
vary and change. For the majority of our cases, the
client and their family have decided to have Caring
Choices continue to provide ongoing assistance and support though face to face contact
to assure that the plan continues to meet the client's
needs. When changes occur, we revise the care plan,
adjusting it to meet the changing needs of the client.
We offer families peace of mind. With our ongoing
monitoring, we offer updates to families with each
of those visits through the convenience of phone
call or emails. Families are immediately alerted
to problems with the goal of providing interventions
that work to prevent a crisis and unnecessary hospitalizations.
We arrange and escort clients to doctors' appointments
providing doctors with necessary information and
provide families with direct feedback.
